Notice of Privacy Practices

In order to ensure compliance with the federal Health Insurance Portability and Accountability Act (HIPAA) Privacy Rules, Illinois State University, Student Health Services has developed a Notice of Privacy Practices (NPP). HIPAA requires Illinois State University, Student Health Services as a covered provider, to make a good faith effort to obtain an individual's written Acknowledgment of Receipt of the Notice of Privacy Practices.

Patients who have not completed the Acknowledgement will be asked to complete it prior to their visit at the Student Health Services. In order to assure that all patients are treated in a timely manner, we request that you arrive a few minutes early to allow adequate time.
