Nutrition Analysis
Nutrition Mission offers free dietary analyses for faculty, staff and students. To have your diet analyzed, please follow these instructions:
- Download the "Diet Recall" form located here.
- Track and record all that you consume for a 24-hour period and record on the Diet Recall form.
- Place the form in the "Completed 24-Hour Recall Forms" slot located in Preventive Medicine Clinic in the Student Services Building, room 291 or campus mail to Nutrition Mission, Campus Box 2540 Student Health Service.
- You will be contacted within 2 days for an appointment.
- Appointments last approximately 30 minutes.
- At that time, we will discuss your results, provide you with suggestions, information and answer any questions that you may have.
Or
- Come to the Preventive Medicine Clinic Student Services Building, room 291.
- You will see a file folder on the wall in the office that says "24-Hour to 3 day Diet Recall Forms".
- Track and record all that you consume for a 24-hour period and record on the Diet Recall form.
- Place the form in the "Completed 24-Hour Recall Forms" slot or send by campus mail to Nutrition Mission, Campus Box 2540 Student Health Service.
- You will be contacted within 2 days for an appointment.
- Appointments last approximately 30 minutes.
- At that time, we will discuss your results, provide you with suggestions, information and answer any questions that you may have.
