Copies of your immunization record on file with Student Health Services are available with a signed consent for release and payment of a copy fee.
Copies are available for pickup after 1 p.m. on the next business day, Monday through Friday. Picture identification is required to pick up your copies. Copies can also be mailed to the address you specify on the signed consent for release. For further information call our Health Information Management Department at (309) 438-3116.
