Illinois law, Public Act 85-1315, requires all persons entering a private or public institution of higher education to provide proof of immunity for Tetanus/diphtheria (Td), Measles, Mumps, and Rubella. Proof of immunity can be submitted by completing the immunization section of the Health History Report or by attaching to this report a copy of the Certificate of Child Health from your high school or, by attaching other immunization records. NOTE: All dates/information must be certified by a physician or public health official. Submit health history report/immunization dates as soon as possible after acceptance to Illinois State University.
- Tetanus/diphtheria (Td): The date of at least one dose of T/d within the 10 years prior to starting classes at ISU. International students are required to provide minimum of 3 dates of DTP and/or Td, at appropriate intervals with at least 1 dose with in 10 years of the first day of classes.
- Measles: Dates of two doses of live measles vaccine given at least 30 days apart, after one year of age and after December 31, 1968 or the date measles was diagnosed and the date certified by a physician, or a laboratory test proving immunity to measles ( lab report must be submitted and can be attached to Health History Report).
- Mumps: Date of one dose of live virus mumps vaccine given after one year of age and after December 31, 1967, or the date mumps was diagnosed and the date certified by a physician, or a laboratory test result proving immunity to mumps (lab report must be submitted and can be attached to the Health History Report).
- Rubella: Date of live virus rubella vaccine given after one year of age and after December 31, 1968, or a laboratory test proving immunity to measles ( lab report must be submitted and can be attached to Health History Report). History of disease is NOT acceptable.
Persons born before January 1, 1957, students enrolled at ISU prior to August , 1989 and those taking only off-campus classes are exempt from this requirement.
Note: In addition International students are required by the university to be screened for freedom from tuberculosis disease within 10 days of arrival on campus.
Any student who has not complied with the immunization requirements by the 15th calendar day of his/her entering semester will be charged a $25 administrative compliance fee and a block put on their registration for the following semester.
